I understand that international travel for pets requires specific and sometimes extremely strict guidelines and protocols from the country of destination. Because these guidelines and protocols vary from country to country and frequently change, I understand that it is my responsibility to research what is required from the consulate of the country that my pet is traveling to as well as from the USDA. I agree to inform Applewood Animal Hospital (AAH) of necessary requirements, will give ample time to complete the necessary paperwork, and will review all paperwork before my pet’s travel date to ensure that all is complete and accurate. I understand that while AAH will make every effort to ensure that all requirements are met, I am solely responsible for all information that is submitted for international travel and will not hold AAH or any of its team members responsible for any incorrect or missing information that may prevent my pets travel.
